To make your first visit as smooth as possible please fill out our Patient Registration Form and the Patient Release Form below and bring it with you to your first appointment. If you're unable to complete out the forms beforehand, please come to your first appointment 15 minutes early.

Payments and Insurance
We accept Checks, Cash, and Visa, Mastercard, and Discover Credit Cards.
Our office is in-network with Anthem, Metlife, and Delta Dental Insurance. We are happy to file other dental insurance policies as an out-of-network provider however this may result in an increase in your patient co-payment. Please get in touch with our office for specific questions regarding your dental policy.
